    sir Ragluf, thanks for the advice nga pla about expired loa. I was able to get an extension. I have a question ulit, if mag fill up ako ng generic form, do I have to put the UCI number that the've written on my previous application? the one that was returned due to expired loa? thanks in advance!

    Thanks fOr the reply Ragluf. Now I'm more worried ::), my loa was valid till sept27 only, then I received my forms on Nov23. I have a friend who suggested me to just send the same forms and include the newly filledout payment form. And so I did, now I already sent my application to CIO...
