Yes you can submit all the documents like, Passport Entry & Exit Stamp Page, Boarding Passes, Photographs, Travelling documents in India, if any....just send all with Cover letter. by the way, had you mentioned any future travelling plan in your application?

I believe, there is no need of any appointment...but you have to check the times. Just call them or chat online, they have options.

Believe me, it was more worse than that...specially when you aren't present in that country. Every country have different law and different format to issue the same. Punjabi ch..bade paped velne paye ;)
