well , I had forgotten to mention my L1B rejection way back in 2008 , and realized it was better to have they updated to the NDVO, hence sent a cover letter and the updaed forms, as advised by a fellow member. Hence would advise you to do as well, NDVO would appreciate on the honesty here...

I would say send the updated forms with cover letter mentioning this mistake, which stage is your in? Have you got your PER. I had a similar issue and shared the updated forms as advised by the seniors on this forum, this was after my MR stage.
