I would be interested in the answer to this too. Additionally, I am curious if arrival/departure stamps are put on the OCI or not. This is important because it does not have many empty pages and is liable to fill up fast if someone travels a lot. Would require renewal in that case I guess.

I had also been using the BLS tracker auto-fill to copy paste the Ref no. onto the OCI tracker. Passport number I had memorized after few tries. As for captcha, I didn't mind it much, as it gave some extra time to pray for a status change before hitting Enter. ;)

My personal thoughts on this. If your mother's passport was valid on the date of application submission, then you do not need to upload/send any documents unless asked to.

What do you understand by 'Permanent Address?' It is usually referred to your place of residence (owned or rented) where you live and can receive mail. It doesn't have to be an ancestral house (in Indian context). So I would advise against using Indian address as permanent address, unless...
