    I find this quite simple because you have not legally changed your name. Q4a. Name as it appears on PR card Q4b. Check NO Q5. Put both names. You can select married name and maiden name from the drop down menu to clarify things.

    This is not correct. You be confusing ei with welfare. You don not need to PROVE that you are actively looking for a job. You just need to be physically present, ready, willing and capable of working during your ei reporting period.
