I think you need to call them, because your daughter is already 16 yrs old. Its mandatory for 14yrs old and above to attend the oath-taking. They will definitely ask for the oath invitation before she gets inside the room.

We have the same test date... I wish my oath-taking is July 1st too, so that it will be memorable. But still overjoyed that I received the oath invitation. :) I hope all the December applicants will get their test and oath-taking invitations soon.... :)

I did the same thing.. All forms in one envelope, no proof or ID whatsoever. Express post. After 5 weeks, we received our records individually or in separate mails.

I got 3 pages.. It was only all the code of the doctor who charged the Ministry of Health. Received it through regular mail.

I applied for OHIP records last April 2014 and received it after 5 weeks.. I also used the form asking for MY personal records, and I did not pay anything. Hope this helps.
