Hi there, I hope someone else answers your question with more certainty, but what I would do in your place, is write the NOA twice since this is what you are submitting as proof and include a note with explanation and photocopies of 2 T4s. I hope this helps.

I am really not sure why you are confused... the IMM5770 gives you two options: Certified check OR credit card.... I dont see bank draft option anywhere....

Cprak0 - I didn't say the link to the form I sent is the form Anthony signed, but essentially the text is very close: you confirm you dont have any trouble with the police etc.
