They fill out and sign both copies of COPR. They give you back one and they send off the other with the photo to CIC. They also stamp your passport. PR card should arrive a few months later to the address on your COPR.

It is odd to receive your complete application package back especially since you received Decision Made on September 8th and you also had a request for passport copy and bio page update. Hopefully it was simply an error and you receive your COPR soon. Wishing you the best and keep us posted.

Yes we took a photo. It is titled "refusal of entry to the United States". It only had my husbands name on it and the comment section was left blank. It is required paperwork when flagpoling. It's nice to know I'm not alone in forgetting to bring the passport :)

I did EXACTLY the same when I drove my husband to land (flagpole) last month. Nothing to worry about for future travel to then US. If your are ever asked about being denied entry to the U.S. say Yes that it was for immigration landing.

I did that last November. If you received the confirmation email right after sending the request there is a reference number and a email address in the correspondence. Fill out the release form, attach it, use the reference number with an explanation. It worked for me :)
