It is asking the addresses where you have lived, so no you need not mention hotel stay of 2 days or vacation outside Canada. But list all addresses where you actually lived (i.e. list addresses where you had residence and you were homemaker or studying or working, etc.)

Yes, no need to fill representative form if u r not using the representative.

I believe that fees payment should be BEFORE you send your documents. There is no reason to pay fees so much in advance.
