Which Open work Permit do i fill out to send in with my Spousal inland and Permanent residence applications. Confused on which one it is.

For the sponsorship evaluation form, do i print the form so its on both sides of the paper? Or do i print the form out as two separate pieces of paper. I only ask because question 6b says to provide details on the back of this page which is the second page.
