I am also April applicant, submitted on 29th April 2022 and still waiting for AOR. It generally takes 3 to 4 months to get AOR.

29th April, still waiting. Mostly online files are getting AOR's after 90 days.

Call them and ask if they requested for medical extension or not. Sometimes they file for an extension, if it passes then there is no need for new medicals. I think in this case the extension might not have passed, that's why they have asked for a new medical.

Because VO must have thought that medical will be expired before the landing process would be completed. Did you friend exited country in last two years ?
