No fund proofs were asked to me. I took bank account 2 months printouts, 2 pay stubs but Only coPR, US admin refusal(in case of no us visa) & valid passport were asked.

@venugopalt : No i did not get any request for PCC after medicals. I had sent 2 diff countries PCC along with my application and 3rd i sent later because i got that in Jan 2014. PCC requests depend on applications timeline as dannymp indicated. @Vikram_saxena : ecas status is still medical...

Thanks veronica79 I have a small doubt. do i need to send photos along with passports? My PPR email does not say anything about photos. :-\ however i read some people have been asked to send photos.

No i did not take my medicine or doctor note with me. I just told the doctor. @venugopalt: you are right the questionnaire is for doctor to fill but they asked me to fill to cut the discussion short. I only took med req form & passport with me. Rest of the things happened at Dr. Office.

I told upfront during medical examination because there is one question in medical questionnaire where you need to tell if you are on prescribed medicine. I left that blank but dr marked it yes & wrote the thyroxine dosage I am on. The doctor explicitly told me not to worry about thyroid.
