Nothing will happen to your application status as both PNP and CIC guys have seen the xerox of your WP and its expiry date. Go ahead and apply for WP renewal/extension with the help of your employer. Parallel to this, file a request for bridging WP request to PNP guys.

Nowhere in the current checklist (for Toronto and Ottawa jurisdictions) have they asked for Personal Particulars form (needed only for Vancouver). AFAIK, here goes Vancouver - Application form + Personal Particulars form Ottawa/Toronto - Application form + Ref. form
