Congratulations man! I wrote the test on Aug 13 in London as well, two days later the status was changed from In Process to Decision Made but it has been two months and I haven't heard back from their office yet. Kinda getting impatient now lol Cheers

I think everyone has to sign that document, it is one of the form Citizenship officials have to complete as part of the interview or identity verification process. If she ticked off "Yes" for the questions that were on that document, then you should be good to go! Cheers
