I am not sure how they process these things however, I landed April 3rd and received my PR card on Monday 23rd June.

I hardly think it's necassary to provide another reference letter. You just need to inform them of your new employer information, company name, address, e.t.c. I really doubt you even have to provide your salary or any other information.

Yes, they had the hole punched in them also. I would not worry about that. Gather all your documents and resend them ASAP so you don't waste anymore time. Pay online and send the receipt with your application.

Same exact thing happened to me. I downloaded the most recent and up-to-date forms and resubmitted my application with those, however, I did use the same PCC and IELTS certificates. I am sure your supporting documents will be accepted with no problem. I will suggest that you pay your fees online...
