I think you should put the city of where you are signing it and currently living, which is also the resident address you gave right? If so you'll have to advise them of change of address when you go back to Canada. I'm no expert so this is just what I 'think', not what I 'know'

We didn't have proof of phone calls, but tones of emails and proof of travel together. This was enough for our application :)

I believe when they ask for your passport it means that it's approved and they will be issueing your visa. And isn't your timeline already stating PR approved : "Quebec approval: August 15, 2009"
