As a software developer, I confidently believe the date is that when the web page was modified since the website designers or developers have been continuously upgrading the content of the website. It has nothing to do with our applications.

yes, from the letter, you should have got a PDF file, print it out and sign on the second page.

I applied for Ontario. In the form, they only asked if I had relatives in Canada and what relationship between us. They didn't even ask for which province they lived. In June, they sent me an email to ask their detailed address, I don't know what they ask that for.
