I just gave her client Id #, File # and attached the document that we got from hospital when she got her medical done.

Email confirmation that the medical record is received by responsible officer.

I added my mother's upfront medical via webform and after 2 months the status of her medical got updated on the portal. I also enclosed the letter we got from hospital as a proof when I got the actual letter from IRCC asking for her medical. I hope this helps. Cheers!

You need Canadian passport, but don't need to submit it. A signed copy of passport is all you need to submit while applying for OCI. They will check for your canadian passport at BLS and return that back. Below is the checklist of documents: https://www.cgitoronto.gov.in/page/oci-checklist/
