I have read that it is better to do the medicals. Many have got the request. Plus yours is more than 5 years... You have already spent x$ already for the process, if not necessary, you are in luck. But if they want it later, then the process will delay by atleast 15-30 days.

My thought is that it doesnt harm to get the medicals done upfront.
