First: yes its fine. Second: yes and it must be under employer letter head with contact details of the company. Third: yes it is required as part of the requirements. Working condition you can say like full time with 48 hours per week of duty. I hope it helps.

I called again and now finally they gave me the temporary file number. Hopefully will get the AOR soon.
