I emailed PHAC approval with all details filled in one email. (IRCC.COVID-TravelExemptions-Exemptionsdevoyage-COVID.IRCC@cic.gc.ca). After few days i got intermarry request in CIC application.

Paid ticket ..

I booked one way and submitted the itinerary. In visa letter they mentioned flight details including time,date etc

What I know is travel should be done before PHAC validity. In CIC Itinerary request it says travel should be with in 30 days or validity of PHAC (which ever is earliest)

Finally got visa approval letter today ..
