Birth Certificate:

1. Miscellaneous services form (see 8 below), duly filled in and signed with a recent photograph
2. Notarised photocopy of first three and last three pages of the current Passport (notarization not required if applying in person).
3. VISA status: Copy of Pmt. Res. Card or work authorization card or copy of F1/ H1/B1 etc
4. Proof of residence i.e. notarized copy of utility bill or Dr Licence or Lease Deed etc in the name of the applicant (notarization not
required if applying in person).
5. Passport(if applying in person) – Consular officer may demand passport for verification .
6. Fee: $ 20/-
7. Processing time: submitted on the counter by 12.30 pm : same day
By mail: 15 to 20 working days from the date of receipt in Consulate (not from the date of mailing by applicant)

Police Clearance Certificate: The process is completed in two stages.
Stage 1:

1. Miscellaneous Services (see 6 below) form, duly filled in and signed (no column be left blank).
2. Notarised copy of passport (first three & last two pages) – DO NOT SEND PASSPORT AT THIS STAGE.
3. Proof of residence i.e. copy of utility bill or Dr Licence or Lease Deed etc
4. Referal Fee: $ 20/- (for seeking clearance from Passport Issuing Authority) – only in cases where passport has been issued from a place other than Chicago. This fee is not charged if the Passport has been issued from Chicago more than 5 years ago. However if the passport issued from Chicago is less than 5 years old, the referral fee is required.
5. Mailing Fee: Express Mail: $ 20/-; Priority mail: $10/- OR adequately stamped self addressed envelope for return of documents. No tracking numbers are furnished for Priority Mail and SASE

Stage 2

1. Misc Services form (see 6 below), duly filled in and signed, clearly indicating the reason of seeking PCC i.e. for immigration to...... etc etc.
2. Passport in original – for making an observation of having issued PCC.
3. Fee : $ 20/- for making observation at 2 above.
4. Mailing Fee: Express Mail: $ 20/- (plus $ 2/- for each addl ppt); Priority mail: $10/- (plus $2/- for each addl ppt)
5. Processing time: Stage 1: 50 days from the date of receipt of application in the Consulate
Stage 2: In person: same day
By mail: 2 to 3 weeks from the date of receipt of Passport in the Consulate

NOTE: While mailing application for first stage, envelope containing application should be clearly marked in bold letters: “Application for PCC”. It will help if e-mail address is indicated on the Misc Services form so as to establish contact, if required. As soon as clearance is received, applicant is intimated for commencing the second stage. PCC is issued within 10 w/days from the date of receipt of Passport in the Consulate. Envelope containing documents for the 2nd stage should be clearly marked in bold letters : “FOR PCC” No queries entertained before the expiration of processing period indicated above.

Hi,

I got my PCC from Indian Consulate Chicago sometime in November last year(2008). The process was as mentioned in the forum by few other members. First we need to submit photocopy of our passport with 1 photo attached in the application for PCC. then pay $20 per PCC(my case was for me and my wife so $40), they gave a receipt and told to come back after 60 days from that day which (the date to came back) is marked in the receipt as well. You go back on or after the date mentioned on the receipt with your original passport and pay another $20 and collect the original PCC, sametime your passport is also stamped with information that PCC was issued on whatever date it is. If you are unable to go with the receipt you can send someone with the authorization letter and original passport to collect the PPC. But the staff at the Indian cosulate are EXTREMELY DSICOURTEOUS, so I personally advice you to go instead of sending someone else as the created a problem to someone before me who came to collect on behalf of someone, hope this clarify's.

WHAT ABOUT THE FEE I PAID EARLIER??? AND WHY SO MUCH TIME???

THEY ARE REALLY CRAP......

I read somewhere on the forum or from Cosulate in chicago that
If you dont get PCC within 60 days, then come with your passport and they will make one and give you on the same day.
not sure if it's true.

Yes what vishals_me says is correct, you can take your original receipt that you were given when you went first time, provided 60 days are over and can get your PCC fromthe consulate.

I have to start the process of getting PCC from consulate .I have renewed my passport from Chicago consulate a few months back as I was in Ohio back then. Now I have moved in California ,can you suggest me if I should go to San Francisco as SFO consulate takes care of Ca state ? Thanks

Guys

Let me help you here.

Do exactly as said in chicago consulate website. Send the application form,passport copy. (No need of photos for PCC... I Personally confirmed this "Speaking to a live person").

Try calling the number, but they wont pick up. But typically they apply for PCC same day they get mails.

Try to get in touch with a lady named shailaja, she will be helpful.
Also try to get the email address for vice consule (he responds in 2 days)

My wife's PCC came in 15 days (mumbai passport), mine hasnt come (bangalore passport).
If the PCC does not come in 60days, you can go to chicago or send passport and get it stamped for PCC.

Considering this is the thread comes up in Google when someone searches PCC from Indian Consulate Chicago, I thought to update it with latest information for others. -

Got my PCC done last week (in person) and here is the latest process.

Get all the required documents mentioned on their website (I had all the documents copies notarized) -

Note that getting PCC is one day process (does not matter where you got your Passport from, India or USA) so in ideal condition you would get it same day. Only exclusions can be if they find anything wrong with your passport (they take your original passport to verify so must go there with the original passport)

Timing to visit consulate is between 9.30AM-12.30PM only, so be careful and be within time frame. Submit your all documents with $27 fee (either Money Order from post office or you can pay in Cash also in the consulate). They will ask you to come back after 3PM same day. Go do some shopping down-town or go for Chicago river walk ... :)
Come back after 3PM and go to the counter directly, collect your PCC and you are done.

It is a very simple process if you go there with all the items mentioned on their website. Initially I was thinking about sending things by mail, but then I would have wasted my 60 days. In person you get it in few hours.

Did you send them your original passport?

I am reluctant about sending my original passport via mail; and will remain tense till I get it back :(

The website says we need to send original passport, but their representative on the phone says otherwise. She told me very confidently that original passport is not to be sent.

Yes, I sent my original passport. I was reluctant too, so I opted for a 1-day delivery service which USPS was offering (I stay 9 hours away by ground travel from Chicago) thinking that lesser the transition happening in mailing, lesser the risk of losing the package. Though the chances of losing the package domestically is pretty low, I felt it is a bit risky. However, I had too many other stuffs to attend to, so I sent the original passport to just get done with it in one go and not live in uncertainty in the meantime.

I recently had to send my passport for CoPR to Ottawa. This was even more risky as it was an international delivery. Again I opted for 1 day delivery but through Fedex and kept checking all the tracking notifications every now and then.

Hi, I just checked Chicago Consulate website as this term sounded unfamiliar. Personal Particulars Form was indeed recently added. I did not have to fill this out when I applied in October, 2017.

Thanks @systix for pointing me to this post. I was little confused by this Personal Particulars Form.
I'm not sure whether the top part of the form should be filled by the person applying or by embassy officials.
Appreciate any help on this.

I'm in the process of sending documents to Chicago embassy via mail. But, I'm little confused about personal particulars form.
They're asking for dates for which I need police clearance in the top part. Did you fill this section?
