Thanks, however all my ID (including passport) that proves I am a canadian citizen are in my Maiden name - hence the question. I have decided to play it safe and fill out all aplications with my maiden name in line with the copy of my passport I am sending.

I am sposoring my husband, since we got married I have not changed the name in my passport to my married name. Should I therefore use my maiden name on all forms where it indicates "family name"? Or am I ok putting my married name even though this differs from my passport. Thanks
