Okay, I understand your question now..... but an I-94 should have been issued with H4 status (at the border)? Maybe I-94 should be good enough. I am not sure they would stamp the passport with H4.

Applied in March and got conditionally approved just last week. So it took around 10 months...

Had my interview today and got approved and surprisingly they did not ask why do i need it. The cbsa officer went through laundry list of dos and don'ts as a nexus card user. couldn't follow much of it (as the she said it like the auctioneers), but as i know few things already i am okay. but...
