In my case everything required except no of hours is present in the reference letters but they mentioned that I was a permanent employee. They are not ready to provide me no of hours thing. Will it lead to rejection?

If I am entering details of PAN card in national id, do I need to provide photocopy of that? Please reply.

Please reply:- My passport and all other documents only have 'First Name' and no 'Last Name' but few of my reference letters have 'Last Name' also. Should I need to mention it in Generic Form for field 'Known by other name'? And should I also need to provide some affidavit to support it?
