I landed on 14th June and haven't received anything yet, and myCIC is still not showing my PR card has been approved. What's the phone number to give CIC a call? Also when people received a request for photos, was that through their email address? Thanks

You don't need the MCCEE exam to apply for Express Entry. You will need to have your ECA (education credential assessment) done by MCC (http://mcc.ca/repository/educational-credential-assessment/) as they are the only organisation who can verify a medical degree for Express Entry

Not sure quite what you're asking - I would think you should fill out the form as they will send the COPR to that address and you have to sign and date the form
