1. number of applicants = principal applicant +dependents(if any)+ sponsor + co-sponsor (if any)
2. Family name enter it in Capitals

Use webform or send as email(probably in your document request it tells you). I raised webform

I did below two things:
For family name- use capitals
For number of applicants- use applicant + dependent + sponsors(sponsor +co sponsor if applicable).
