hi there, from what i have read on this entire forum, i believe you do have to fill that form regardless of any changes. so i guess you will put the same information as you did before. but i hope someone answers your question with exact info.

applied in september 2013 received application in october 2013 1st medical in november 2013 quebec selection feb 2014 file in process jan 2015 remed 5th of may status changed to DM last week
