If the request is from India then below are the documents they asked Original Travel Document say Passport. Any Medication if you are taking that prescription. Upfront Appointment letter. Immigration form.

No you don't have to do anything. Hospital will send the reports directly to IRCC. Just make sure that they have sent it. Hope they took the medical from the hospital mentioned in IRCC website
