Yeah its just your mom who has to fill the form... and get it signed ... mention your and your sister name in section - B (for children)

I got it first time.... however the confusion is she does not have any dependent ( my wife's brother is 27, cannot consider him dependent). Am I suppose to just fill the 5406 for her mother ... is that correct?? ================================================================================...

The form needs to be filled by your Mother (Principal applicant) and if any dependent.
