My daughter’s application was sent by my husband from inside Canada and yes CIC send the certificate to my husband’s address directly. However, it depends, if you apply from outside Canada, they will send to the consulate and you can pick the certificate there.

I think you can give your old medical record and provide cover letter. For spousal sponsorship, as far as I know, you don't need to be in status. You need the status only for OWP. No worries, OWP usually has received date 1 week before the real date they received it. I dunno the reason...
