We have the same situation. It just i didn't used my caucasian husband last name. I just legally apply for name change due to divorce. So i just remove my ex last name and back to my birth last name. In your case you can ask vital statistics you can find an exact answer from them. Goodluck

Hi Russell, Its ok even the Philippine passport is different from your current name. If you travel to Philippines or anywhere they said just show your name change certificate.

Doesn't need police report just apply verification of status and what i did i explain why i lost it. You need to pay 30 dollars and they will send to you through email.

You don't need citizenship certificate just the birth certificate of the baby. You need to apply online, mine it takes a month to arrived the birth certificate then i apply for passport it arrived after 3 weeks. I think you can ask them for urgent processing coz they ask me if i have travel plans.

You need to be at least 2years as a permanent resident before you apply.

Did u call cic that you want to change or it just updated itself after you change your name legally.
