I personally do not think it matters on the format of NA or N/A. As long as you do not leave any section empty and it's clear to them that it's not applicable to you.
Question 6B is for people who lived in Canada before their landing date. So if your mom had lived in Canada prior to 2010 then use that date. Otherwise it's the same date for both 6A & 6B.

I totally agree with you...cases get delayed for no apparent reason. It's probably because they have such a huge backlog of applications and it's work that will eventually be looked at and some of us just get stuck waiting longer in the entire process then others.

I don't think it's good to have the incorrect name on file...I would either send in a fax/inquiry because your sister in law doesn't want her Visa and paperwork to have the incorrect info on it. It should match her passport information.
