can't say for sure about the time, I checked this thread in morning and saw people with similar timelines has got DM on ECAS, so I decided to check mine and it said DM.

Pre Arrival letter is a document that CIC sends you, some people believe that once you get that document your eligibility is cleared, also not everyone gets that letter so it's not that important.

Got DM on ECAS today. Waiting for PPR. Did anyone who got DM on ECAS today, got PPR email?

When you will get medical extension, it will say you passed the medical on a new date, till that time it shows you do not need a medical exam.

for Jan/Feb people who are getting their PPR, do you just get the email? or is there something that is updated in online account as well?

Yes, having said that, it assumes that you already have a PNP and this is just for federal paper-based PR application, then NOC skill level A or B should bypass IELTS.
