I applied for passport by certified courier to to Passport program, Gateau, QC, J8T8R1 ... but by tracking. it was delivered in Ottawa, and not in QC .... did you have same scenario ...also do receive any emails confirmation from the passport office... thank you

No , you choose one of them... regarding the form you can write your name only ... signature and date must be in front of the officer... good luck

Do you know that they want the original copy of the citizenship certificate or They accept a notarized copy?????

You write your name only than you fill the two spaces for signature and date in front of the officer
