I think you'll have to address it yourself, unless you have a US address in your name. But it's worth checking with the rest of the board.

That's correct. But always best to err on the side of caution. Give them all the documents they need, so you don't waste time with the CIO asking for additional documents. Merge it all into one file, with separating placeholders/dividers if you wish.

Cover all your bases is my suggestion. Provide the certificate in the original language, along with a certified English or French translation.

Yes, as long as you provide some kind of proof that you applied for the clearance in a reasonable time period, they will be fine with this. Courier receipts, etc. Just be proactive, and use the LOE to make all explanations.
