There was a question somewhere in 5257E asking if you have every been refused a visa or permit before and the other is if you have previously applied to enter or remain in canada. You can state the complete uci there too.

Hi bellaluna! :) the xx-xxxx-xxxx pattern specifically says it is the uci. It is purely composed of numbers in that pattern. Opposite it is the application number. It is alphanumeric.

Hi bryanna The forms are the ones i downloaded from the site. Those are also the forms that are required to be attached for online application. Without those forms the "continue" button will not appear.
