Hi all. The NBI clearance form is confusing for married women. It asks for family name and husband's surname. Not sure what to put in the surname box. Whether the married surname or the maiden last name.

Did anyone found the application form confusing for married woman?

Thanks. I've seen this in the guide. Permits are to show status. I think I'm going to add more documents to prove entry since the guide also said "you will need to prove that you ... are living in Canada when you submit your application and when IRCC makes a final decision on your application."
