Sorry dude but I'm not following you.. The information sheet and medical report was provided by the panel physician. I don't know what patch notification is - the regional medical office? Interestingly I myself had to do additional exams for a preexisting condition. The date after I submitted...

This is the first medical. Though we did do another UME for a previous PR application 2 years ago, where she received an A grade

Hi Slimmy111 No, the IRCC only accept TCF Canada for PR applications (or TEF Canada), no other variations are accepted. Bon courage
