Why do you need a "spouse name" in a passport?
Never heard of that in a Canadian passport.
So the answer is spouse name is not mandatory or exist.

Perhaps OP is using the maiden name and want to see if the spouse's last name is mandatory in the passport. No, this is not mandatory. OP can continue to use the maiden name.

Thanks for bringing that up! I was thinking about a field like "spouse name" with the spouse last name and first name in a passport.
But agree that OP can use maiden name because it's freedom to change to husband or wife's last name or not after one gets married.
