Do I fill out just one for the both of us (Me+Wife)? or Do I fill one out for each of us? also if I do have to fill out the form for each of us, would i be considered the applicant on the form about myself? Please help and Thanks!!

The principle applicant fill the additional family information. And since it's a sponsal sponsorship, the sponsor's name will be in it (under spouse).
