Hello All
I received a quality assurance check documents and I need to provide provincial health claims form. I was in Ontario for 2 years and now I am in Alberta. For the Ontario health claims form from OHIP, I saw this link which doesnt have any standard form to fill out , I just need to write a letter. (http://www.health.gov.on.ca/en/public/programs/ohip/phi_access/default.aspx)

There is one more link that I saw: http://www.forms.ssb.gov.on.ca/mbs/...tFileAttach/014-4890-84E~1/$File/4890-84E.pdf which gives a PDF form to fill out but the mailing address is always set to ON and I cannot change it.

My question is:
1) To get a Ontario health claims summary, do I just send out a letter providing name/mailing address etc and would they just mail me back the document ?
OR
2) Do I fill up the PDF (shown in second link above) but the mailing address cannot be set to anything other than ON. and I am in AB now.

Any help is greatly appreciated.
 
I have the same issue. I need to get Ontario personal health summary but I live in Quebec now. Anyone have any insights on how to fill the form for out-of-province people?

Hello there,
You could try getting Pharmacy Rx prescription list from your local pharmacy as it's not easy to get Quebec health records in 30 days. Do verify with the Immigration Officer if that will be sufficient.

I am in the similar situation as you and I just called them at 1-800-262-6524 ; it brings you to voice message. It says that as of today they are processing applications submitted on Dec. 4th. Which I assume means that it might take them longer than 30 days to send the personal claims history.

Also, as related to your question, this is the info on their website:

Please Note: In order to process the Request for Access to Personal Claims History (PCH) Information by Individual or Individual’s Substitute Decision Maker, the individuals' health card mailing address must be up-to-date and must match the return address on the request form. Please contact ServiceOntario to update your address prior to submitting your request, if required.

Update: Just called them again, and the person on the phone confirmed that they will only send the form to the postal address that is indicated on the individual's health card.

Wow, I can't believe they will not send the summary to another postal address. What if the person moved to a different province?

My understanding was that they will be able to send it to a different province as long as that address is on your current health card.

Did you cancel your OHIP? For privacy reasons they only send things to your address because if not anyone could order your records if they had your basic information.

I moved to a different province so I believe it was automatically canceled after a while
