i think we need to send on excel file . Why we have only LA option in that . Can we write New york in the same file

Yes you can.
I did that because I haven't heard of anyone who submitted it there, and just to be safe. But under the Application Type, I chose 'Original Passport Submission'.

Just got off the phone with them, and they said 'Original Passport Submission'.

If in doubt, feel free to contact them and ask. The wait time on the phone was only around 5 minutes before being connected with an agent.

In consent form do we need to send 3 and 4th page.
->I am sending myself so no agent
->I am done with my biometric
I asked about that, and told them I got biometrics done at USCIS office, they said I could leave the last 2 pages blank and only fill out and sign the second page.

Can someone please confirm and let me know if i am missing something (or Do in need to make any payments other than VFS courier charges and TT service charge)
1- Send email to VFS for 2 way courier + TT service charges ( They said it would come around 82 Dollars = 61+21)
2-Original passport
3-OPR letter
4-My address on a paper
5-Payment receipt.

Yes, thats the amount you have to pay and also what you have to submit.

Congrats to everyone who has received their OPR. To those still waiting,just be patient;it will come. I received mine,too, yesterday.
Do I have to pay for the two-way courier services(label) for my dependents (four) too? I guess one package should be good for my four dependents and I. Afterall, if I was allowed to use either UPS or FEDEX, I could put all the five documents in one package and mail it.
Is there anyone in here in my situation: having at least a dependent? Did you pay for every dependent or you only payed for yourself ( the principal applicant)? I will be glad to hear from an experienced individual in this regard.

Again, I was a bit surprised when I received the Original Passport Request. The reason is that I already have a TRV earlier this year. Initially when the mails were rolling in yesterday, only those of my dependents came. Hence, I thought I wouldn't be required to submit my passport for stamping because of my active TRV. However, about an hour later, I received my OPR. I am not sure if they would want to stamp the passport a second time again. The passport might just be returned unstamped despite being made to pay for it. Who has been in this kind of situation before?
