Transit visa

Hello, I will be in Toronto for 4 hours in a way and when I return I will be 24 hours in accordance with this, I have to apply for a transit visa.
I am filling up the IMM 5257E FORM. I have some doubts.

I selected: Transit Visa

1- PERSONAL DETAILS: When i have to fill up "city of birth" It isn't written in my passport. What should I write?

2- DETAILS OF VISIT TO CANADA: I am not enable to select the dates that I am going to stay at the airport when I go and when i get back. I only have a couple of dates to select.

I would be absolutely grateful if you answer me.

1. You should know what city you were born in.
2. You need double entry transit visa..
