So if i have been driving truck for last 4 months but within last 5 years i have completed 3 years presence. Do i need to mention all my in and outs of canada ? What if i just say yes in question 14-b and then do i still need to mention every entry ?

Yes - you need to mention every entry and exit.

Isn’t it like if i have been only driving for 4 months usually they can get the history as it says in the form question 14 part b ? It says “i hereby give my consent to the Canada Border Services Agency (CBSA) to disclose the details of my history of travel to IRCC in order to assist IRCC in determining my citizenship eligibility. I also authorize IRCC to collect the history of my travel from the CBSA.” ?

CBSA does not currently track exits from Canada, only entries. So you still need to provide a travel history.
