Hello all,

So I just received an IMM 5669 request. It says the following:

This is in reference to your application for permanent residence in Canada. In order for us to continue the review of your application, we require the following documents:
UCI - John Doe:
>Schedule A: A separate and newly completed Schedule A Background Declaration form (IMM 5669) bearing an original signature.


Q1) Do only I need to fill the form, or does my spouse (non-accompanying) also need to fill and sign her form?
The letter explicitly says "UCI - John Doe" and when I go to GCkey docs upload, an upload option is active under my name only.

Q2) I stayed at one address since birth, but went abroad to study from where I made frequent trips to my home address. I've mentioned only two addresses in my address history. Is this acceptable?
It looks like this: From birth to Current: Address 1, From 2015-MM to 2018-MM abroad address.

Q3) I made two short visit trips (tourism) (7 days and 14 days to two different countries). Do i need to write their addresses as well? I stayed in hotels. I had already disclosed this information in the travel history form.

Q4) Now if suppose, I am to write the (hotel) addresses where I stayed during tourism trips, will this raise issue for me for not disclosing those addresses before?

Your help is much appreciated.

In the ADR it says who the ADR is for. In this case if only your UCI and name are mentioned, then you have to provide Schedule A only for yourself.

Yes, that's fine. You don't have to disclose the trips between your country of citizenship and the country of your residence

No, you don't have to provide any details on your short trips. In fact, you shouldn't even mention your trips there. Schedule A asks only for your address history. There is a separate form for travel history but you weren't ask to submit that

Two more questions if you could please answer:

Q1) In my PR forms I only mentioned my Bachelor's and Master's degree. However when filling the IMM5669, I see I need to enter details from secondary education. I've included those details. Would it be a problem that I didn't include those earlier?

Q2) Since I filed my eAPR, I got a job (previously unemployed). So I've included that detail in the personal history, and also included the secondary education details which stretches my personal history back to when I was 14 years old. Is it okay to give additional info. in the personal history?
