i need to ask that for WES my passport has name GURPREET KAUR as given name .. BAINS as family name and my signature is GURPREET KAUR only ...
moreover the academic degrees has name GURPREET KAUR only ... do i need to get notarize the document that person is same ?

For getting WES assessment. You don't need notarised letter. They will assess your credentials and in the report they will write. Name on certificate and passport is different.
For EE profile. You need to submit the nitarised letter of one and same name letter

Yes. Preferably follow passport name. As you will be using that for EE process.
