IMM5669E Schedule A Parents & Grandparents - who should be filling it out?

Hey guys
Who is supposed to fill out IMM5669E Schedule A when I am sponsoring my mom?
What I mean is the part 1 & 2 says: Personal Details and 3 & 4 says Personal details of your parents.

Should I fill out 1 & 2 and 3 & 4 my mom, or 1 & 2 my mom and 3 & 4 my grandparents?
