We have waited almost 16 months no interview, no landing fee and expired police check and medicals. How long can this go on for?

You should order your GCMS notes to find out the status of your application and what may be causing the delay.
The CR application left the managers desk as it was there from March or April. September it is now with the processing visa officer for further review.

Hi! I got transferred for further review as well after SA in May, the reason was to verify genuineness of relationship. I got a fingerprint request in June after I involved my MP and got a landing appointment letter yesterday. No AIP and Dm letter. They extended my medical as well since it expired in June. It took 5 months from transfer to landing appointment. Patient is key and like the others said order your notes so you will be ready if they ask for clarification. No kids together yet, I'm bringing up my step child and spouse, married 2 years and been together 4 years, no age gap. Only red flag is a criminal record for the applicant. We applied together for family class and CR in the same envelope. We coming up to 16 months for both applications as they were applied together.
Criminal record can be a big red flag and certainly cause delays. What is it?
Both applications for PR and CR we submitted together in June 2017 sent to Mississauga. The PR application has been in process since Aug 2017 at the visa office. Mississauga told me they sent the CR to the visa office Dec 2017. Visa office apparently received it Feb, 2018 and was in queue for the migration manager to review. I emailed the office sept 2018 asking for an update and now the file has left the migration manager desk to the visa processing officer.
