I received my ITA while in Canada, but will be returning to the UK next week as my work permit has expired. Under the travel history it says to include travel outside of your country of origin and residence. When I submit my PR application my residence will be the UK, so should I include any travel between Canada and the UK for the last 2 years? I also studied for a year in the States, hence living there, so should I include any travel between the UK and USA for those dates?

Yes, you should include everything. And I think that you should start preparing the police certificates for anywhere that you have been for more than 6 months. Especially the US one, I have heard that it takes a long time to process (i.e. months).
